"April 16 No. 152, Matilda Kelch of William and Elizabeth Hunt, abode Teynham, occupation: Carpenter, ceremony performed by J Winter" with a note under Matilda's name 'Baptise Nov 19th'
Entry is between those for baptisms on Apr 16 1820 and May 21 1820, so extra note can't refer to 19 Nov 1820, unless entered later. I think the entry refers to a reception into the church in April, following a private baptism the previous November. It could, though, be a private baptism in April 16 1820, and a reception into the church in Nov. 1820, but that would mean the event being wrongly described.

Although the age is wrong in both the registration index and burial register, I am sure the death was this Matilda as her husband married the following year as a widower (his less common occupation along with everything else - age, father etc. - fitting make it clear it was Matilda's husband)
Cause assumed, as the death was registered the same qtr. as the birth of her youngest daughter.
