I guess he was in the workhouse sick room, as he wasn't born in the workhouse.
'Aptha' seems to be ulcers in the mouth. I imagine the wasting, which according to his death registration started when 4 days old, was caused due to the ulcers preventing him taking in sufficient milk. "Neonatal oral aphthous ulceration of the palate also known as Bednar's aphthae is not an uncommon presentation. They clinically present as spontaneously regressing, shallow, and symmetrical ulcers on the posterior palate of newborns from 2 days up to 6 weeks of age." (from this article.
The ulcers or inability to feed must have been sufficient to endanger his life by the time he was just two days old, as he was privately baptised then.
